Jenkins Academy of Applied Technology · DOT-101 · v1.0.0
Employer workplace connection
Course developed by Todd Jerome Jenkins, MS, CSP.
Use fictional facts for classroom practice. Actual medical, personnel and complaint records belong in authorized secure systems. This worksheet does not submit anything to the employer or an agency. Entries remain in this browser page only; print a completed copy before closing. Blank forms and course scores do not verify workplace implementation.
| Component | Separate employer evidence |
|---|---|
| Actual employer policy and driver receipt | Provide and explain all required 382.601 content, identify the question contact and obtain the actual signed receipt; Academy completion is separate. |
| Testing and response arrangements | Verify qualified services, DER authority, after-hours contacts, transport, correct test reasons and prompt decision/collection handoffs. |
| Separate reasonable-suspicion supervisor training | Verify actual 382.603 training of at least 60 minutes on alcohol plus 60 minutes on controlled substances, including required indicators. DOT-101 does not meet this requirement. |
| Queries, consents and reporting | Assign trained authorized administrators, verify consents, deadlines, statuses, accurate evidence and actual reporting duties in the official system. |
| Confidential records and eligibility controls | Verify restricted access, category-specific retention, removal/return-to-duty/follow-up evidence and actual license eligibility before dispatch. |
Explain and demonstrate understanding
Use the actual employer procedures, knowledgeable reviewer and understandable language. Have the learner explain the actual policy, testing and escalation decisions using fictional facts; correct gaps before relying on the arrangement. Record the actual instructor, learner, date, topics, evidence, corrections and limits in the employer’s secure system.
