Fall Protection · Online knowledge course resource v1.0.0 · October 21, 2026

Save entries: this form does not automatically store data. Print or save the completed form as PDF before closing. Blank fields mean evidence is pending. A signature or online result does not approve equipment, an anchor, a rescue method or work outside the evaluated assignment. Keep personal medical information out of shared records.

Federal construction Subpart M worker knowledge core for conventional protected access, guardrails/covers and reviewed personal fall-arrest arrangements. Actual task/system instruction and safe skill verification remain separate. It does not qualify a competent person, system designer or rescuer, or authorize specialized work/alternative protection methods.

Use protected ground-level or otherwise safe arrangements. Do not expose a learner, drop/suspend a person, create an unstable surface or conduct an unplanned rescue for an exercise. The trainer/reviewer must have the actual task-specific qualifications and authority required. This is an Academy evidence tool, not a substitute for design or the employer’s instruction.

Observed explanation / safe demonstrationResult / evidence / date / correction
Recognize task and rule
Identifies actual activity/track, task-specific protection, hazards/support and prohibited/outside-scope work.
Protected access and collective systems
Explains actual guard/cover/access arrangement, how to report defects and who installs/alters/removes it safely.
System/component identification
Identifies actual system purpose, harness/device/connector/anchor IDs, user/load limits, compatibility and manufacturer conditions.
Inspection and quarantine
Performs actual permitted pre-use/shift check, recognizes sample defects/impact indicators and follows reporting/quarantine.
Harness fit and connections
Safely dons/adjusts actual equipment and demonstrates specified attachment/connection in a protected area; no suspended-person test.
Anchor/clearance/swing limits
Locates approved setup and explains allowed area, fall path/clearance, edge restrictions and when technical review is needed.
Access and transitions
Demonstrates approved protected access/transfer sequence without any uncontrolled disconnection or unverified hook combination.
Handling/storage/people below
Explains actual tool/material handling, overhead/falling-object protection and equipment care/storage.
Specialized equipment or methods
Explains assignment boundaries; any used net, roof method, dockboard or special access has applicable specific instruction before use.
Rescue and alarm role
Locates current plan/contacts and explains the learner’s actual role, backup, hold conditions and no improvised rescue.
Changes and skill gaps
Recognizes retraining/change/defect triggers and knows how to stop/report before continuing.

Attach the core knowledge record separately. Online passed does not close actual training/skills, technical protection or rescue evidence.
