Employer workplace instruction and verification
HC-001. Online knowledge course resource v1.0.0. October 20, 2026.
Use this worksheet to connect the online knowledge component to actual chemicals, operations, controls and worker roles. This form is an Academy evidence tool, not a federally prescribed HazCom certificate. The employer determines applicability and retains evidence under its record arrangements.
The fields are intentionally blank. No employer-specific information has been supplied or verified. Blank or unresolved fields must not be treated as completed workplace instruction.
Save your completed worksheet: entries are not stored automatically. Print or save the completed form as a PDF before closing this page.
Applicability and employer preparation
- Confirm jurisdiction, employee tasks, normal use and foreseeable emergency exposure. Review special operations and exemptions using actual activities.
- Check manufacturer/importer responsibilities separately. A plastics manufacturer may also have supplier duties that this employee-use course does not teach.
- Supply the actual labels, current SDSs, written program and chemical list. Reconcile missing or inconsistent information and test shift access.
- Identify PPE, respirator, substance-specific, emergency-response and other related training needed for the tasks.
- Arrange a qualified trainer and understandable delivery. Resolve questions and access barriers before accepting completion.
Observe and discuss
For each area, mark Demonstrated, Needs correction, or Not applicable with a reason. Record what was observed and any corrective action. These response labels and demonstrations are Academy design choices.
| Verification area | Result / evidence / correction |
|---|---|
| 1. Identify actual chemical operations and relevant physical, health and additional covered hazards. | |
| 2. Retrieve the written program, chemical list and correct SDS using normal access on the assigned shift. | |
| 3. Explain an actual shipped label and the employer’s workplace labeling system, including portable-container rules. | |
| 4. Find actual SDS sections 2, 4, 7, 8 and 10 and explain information relevant to the assigned task. | |
| 5. Explain actual exposure routes and demonstrate approved task controls and assigned PPE within the worker’s training and authority. | |
| 6. Identify release indicators, alarms, reporting contacts, withdrawal actions and response limits. | |
| 7. Explain nonroutine-task and unlabeled-pipe briefing arrangements where applicable. | |
| 8. Explain contractor / multi-employer coordination where relevant: SDS access, precautions and workplace labels. | |
| 9. Explain what changes, missing controls or unanswered questions require a pause and whom to contact. |
Remediation and reassessment
Accept workplace verification only after applicable instruction, understandable delivery, trainer questions and observed task requirements have been addressed. A learner acknowledgment by itself does not establish comprehension or working controls.
Online knowledge result: attach the LMS record separately. This worksheet does not alter the SCORM knowledge score or automatically establish employer compliance.
Change and transition review
Review new hazards, revised SDSs, product substitutions, process changes and changed exposure methods. Do not assume a universal annual federal HazCom refresher mandate.
Under the current federal transition provisions, substance-related employer updates are due as necessary by November 20, 2026, and mixture-related employer updates by May 19, 2028. Review actual supplier information, applicable transitional options and any State Plan requirements before implementation. These dates do not replace the initial-assignment or new-hazard training duties.
