HCP-001-CON — Employer workplace component

Hearing Conservation and Noise · v1.0.0 · October 21, 2026

Actual construction task/noise assessment, effective controls, suitable protection, individual inserted-device fitting or determination by competent persons, task instruction, safe communication and any applicable adopted audiometric program remain separate. Confirm State Plan, client and employer additions.

Blank means pending. This local form does not save entries to an account. Print or save your completed employer copy. Record actual observation, correction and reassessment. Keep audiograms, hearing thresholds, diagnoses and clinical findings in the authorized private process. An LMS pass, appointment or signature does not establish actual exposure, fit or a hearing-test result.

Keep completion states separate

ComponentRequired actual evidence
Actual task and exposure assessmentEmployer confirms activity and jurisdiction, representative noise exposure, shift/task durations, peaks, adjacent work and applicable criteria. Link the actual assessment and JHA/task plan.
Effective controls and task limitsEmployer implements and checks the applicable source, engineering and administrative controls, task-duration limits and arrangements for changed conditions.
Actual protector selection, fitting and useEvaluate attenuation for the actual environment and device, demonstrate fit with other PPE and confirm care, replacement and communication. Construction inserted devices need individual fitting or determination by competent persons.
Applicable audiometric program and follow-upAuthorized provider/employer process establishes actual testing, dates, valid results, private notification and indicated follow-up under the applicable rule or confirmed adopted construction program. A quiz or booking is not an audiogram.
Actual workplace instruction and correctionUse understandable task and device instruction, worker teach-back, fitting observation and corrective reassessment. Apply the actual annual/update requirements. Keep trainer access and response arrangements clear.
Separate employer and confidential recordsRetain the actual exposure, fitting/use, training and applicable audiometric records through the proper controlled processes. Keep audiograms, diagnoses and clinical details out of ordinary LMS records and public discussion.

Use the actual work

Map industry/NAICS, employee role and actual tasks to the JHA, controls, equipment, applicable requirements and training. Arrange knowledgeable instruction, appropriate questions and enough actual-device practice in a safe setting. Use teach-back and observed fitting. Correct a critical gap and observe reassessment before allowing reliance on the device.

Academy discussions are asynchronous and have no guaranteed response time. The employer must establish the appropriate workplace support route. Use the actual site emergency procedure for emergencies.

GI timing follows 1910.95. For construction, confirm the actual adopted program and applicable State Plan/client requirements rather than describing every GI deadline as construction law. No universal OSHA course hours, score, training certificate format or annual hearing-protector fit-test mandate is asserted.

Federal core only. The employer must confirm actual work, applicable State Plan and other requirements. NIOSH material is guidance; it is not substituted for OSHA’s legal criteria.

Development inputs: training inventory TR-009; SHP-008_Noise_Exposure_Hearing_Conservation_Program_v0.1.html, approved development template, current Library version 1; RAVS_Requirements (41).pdf, exported July 22, 2022, ten unique historical review questions. Historical employer answers, page numbers and review approvals are not transferred. Template approval is not employer adoption or course validation. The historical Cal/OSHA references need current jurisdiction-specific review.
