Course developed by Todd Jerome Jenkins, MS, CSP.

Requirements awareness only. This is not a 40-hour, 24-hour, eight-hour refresher or supervisor specialty course. It supplies no supervised field days, HAZWOPER field certification, medical clearance, fit test or emergency-response qualification. Complete required training and actual site preparation before work.

Complete this worksheet using the actual workplace and current plan. A blank form or course score is not evidence that the action occurred. Record actual dates and observations. Print or save a completed copy before closing; entries are not automatically stored. Keep private health details out of this worksheet.
