HZW-002-ER — Employer workplace component

HAZWOPER — Emergency Response Requirements · v1.0.0 · October 22, 2026

Course developed by Todd Jerome Jenkins, MS, CSP.

Requirements awareness only. This is not an eight-hour operations course, a 24-hour responder course or a complete annual refresher. It does not qualify or certify an awareness, operations, technician, specialist or incident commander role. Actual required training, demonstrated competencies, employer certification, medical/respiratory preparation and authorization remain separate.

Complete this worksheet using the actual workplace and current plan. A blank form or course score is not evidence that the action occurred. Record actual dates and observations. Print or save a completed copy before closing; entries are not automatically stored. Keep private health details out of this worksheet.

ComponentActual evidence
Coverage and policyDocument the actual jurisdiction, evacuation-only or response policy, incidental-release limits and applicable HAZWOPER pathway.
Role qualificationVerify role-specific training or permitted experience, all required competencies, qualified instructor/evaluator, applicable hours, employer certification and annual refresher or competency methodology.
Operational readinessVerify the actual written response plan, outside coordination, command/safety authority, monitoring, PPE/respiratory program, medical surveillance/support, backup and decontamination.
Environmental notifications and transitionVerify EPA/State Plan worker coverage, actual notification triggers and contacts, assigned reporters, post-emergency cleanup pathway and waste/discharge arrangements.
Evidence and authorizationRecord observed performance, gaps, corrective actions, protected health-record handling and employer authorization/limits. Academy completion alone does not qualify a responder.

Use the actual work and plan. Arrange clear instruction, questions, teach-back and suitable safe practice. Correct a critical gap before relying on the arrangement. Academy support is asynchronous and is not an emergency contact route.
