Save entries: this form does not automatically store data. Print or save the completed form as PDF before closing. Blank fields mean evidence is pending. A quiz, receipt or signature does not establish actual protective performance, fit or use ability. Keep medical details out of ordinary shared training/equipment records.
Federal general-industry core for assessed eye/face, head, foot, hand and related general protective-equipment tasks. Actual task assessment, selection, individual fit and demonstrated proper use remain separate. Respiratory, electrical, hearing, fall, biological and other specialized duties require their own applicable instruction/programs.
For PPE covered by 1910.132(f), the employee must demonstrate understanding and proper-use ability before work requiring it. Use actual selected equipment and clean simulated/protected arrangements; never use a hazardous exposure, moving-machine catch test or contaminated item as an exercise.
| Explanation / safe demonstration | Observed evidence / result / correction / date |
|---|---|
| When/why protection is needed Identifies actual task hazards, other controls, remaining exposure and when PPE is necessary. | |
| What equipment is required Selects the actual approved items for the task and identifies model/performance/limit references. | |
| Limitations Explains actual chemical/mechanical/thermal/other limits, change-out criteria, entanglement issues and specialist boundaries. | |
| Inspection/rejection Checks actual equipment safely against manufacturer/employer criteria and explains defect segregation/report/replacement. | |
| Individual fit and compatibility Adjusts actual suitable size/style, maintains coverage/fit and safely checks task motions with all required items together. | |
| Don, adjust and wear Demonstrates the actual taught model/combination method in a protected area without exposure. | |
| Doff and handle Removes clean simulated items safely, explains contamination containment/hygiene and uses the correct cleaning/disposal route. | |
| Care, maintenance and storage Identifies actual approved methods, permitted servicing and conditions that require removal. | |
| Useful life and disposal Explains actual service/change-out/retirement limits and approved disposal/servicing, without using appearance alone. | |
| Provision and stop/review Locates issue/replacement contacts, explains when to pause and obtains help for changed hazards, poor fit or uncertain protection. |
Do not close a safety-critical gap with only a quiz score, receipt or signature. Attach the online core result separately. Record observable fit/use needs without medical diagnoses.
