Course outline
For supervisors and developing EHS practitioners. Start with the investigation purpose, then work through evidence, explanations and follow-through. No prior Academy course is required; follow your employer’s procedures and involve qualified specialists when needed.
Module 1: Starting an Investigation That Can Support Learning
Set a useful investigation purpose, respect response boundaries, and separate evidence from early explanations.
Module 2: Evidence and Scene Preservation
Preserve changing scene information, document limits, and make an accountable evidence handover.
Module 3: Interviewing and Witness Information
Prepare interviews, invite open accounts, clarify uncertainty, and check the record with the participant.
Module 4: Event Reconstruction
Build an evidence-linked timeline, retain competing sequences, and identify focused follow-up and specialist boundaries.
Module 5: Testing Explanations
Compare competing explanations against evidence, revise unsupported claims, and define focused follow-up.
Module 6: Systems and Human Factors
Examine actual work, information, handoffs and control performance without substituting blame labels for evidence.
Module 7: Reporting and Records
Prepare traceable reports, corrections and handovers while keeping legal reporting and private records within their proper processes.
Module 8: Corrective Actions and Verified Learning
Select condition-specific actions, assign accountable owners, and verify implementation and effectiveness.
Module 9: Patterns Across Events
Check event data, denominators and recurring conditions, then propose a focused control review.
Module 10: Specialist Boundaries and Integrated Capstone
Prepare a specialist referral and an integrated evidence, action and learning handover.
