Safety climate at your workplace

NOSACQ-50 English 2019. No account or purchase is required.

2026-Q4 · Calendar window begins 10/1/2026; closes immediately before 1/1/2027 at midnight Eastern. Please answer once for this workplace in this quarter. The quarterly protocol first launched October 26, 2026 (Eastern time); its first quarter has partial coverage.

The regional label identifies the questionnaire edition, not your origin or citizenship. The research team’s Spanish instructions and consent were drafted with AI assistance and have not received independent bilingual review. Cross-language equivalence for this U.S. sample has not been established.

Company code · optional

Have a code from your workplace? Check the company below before choosing to include your answers in its grouped report. Leave this blank to participate independently. No personal account is required.

Company-linked responses contain a company survey reference, but no name, email or personal account ID. Shared codes can appear in hosting logs and do not verify employment or prevent repeat participation. Employers receive no individual answers or completion list. Reports require at least 30 usable responses in every dimension, quarter close, a seven-day buffer and research-owner privacy/quality review. English and Spanish are kept separate. Comparisons use other newly consented participants in the same quarter/language, across all industries, excluding responses linked to this company. Small groups are withheld. Rare combinations and information known to coworkers can still create identification risk. A released grouped report cannot later be recalled one answer at a time.

Workplace context · optional

Think about one current workplace or work unit throughout. Do not enter names, employer names, exact locations, incidents or medical information.

Choose your industry context

A sector page preselects its sector. Choose a more specific industry if you know it, or keep the broader sector.

Industry context is included only if you submit the questionnaire. It does not update your company profile or give research consent.

Classify the employer’s work establishment, rather than an individual occupation. Change the sector if your work unit belongs elsewhere. A classification does not establish regulatory applicability.

Choose the state or U.S. territory of the workplace you are describing, rather than where you live or your employer’s headquarters. If you work in several states, choose where you mainly work, or Multiple states. State and territory names are shown in English. We use this optional answer to understand geographic coverage. It stays in the private research response and is excluded from company counts and employer reports. Rare combinations may make someone recognizable; any published state summaries require privacy review.

Citizenship · optional research question

We want to explore whether U.S. citizens and workers without U.S. citizenship describe different safety experiences, and identify possible gaps in support to investigate. This answer stays in the private research dataset and is excluded from employer reports and company participation counts.

Do not provide nationality, immigration status, work-authorization details or documents. Citizenship is different from those topics and from language or birthplace. Skipping has no effect on participation, account access or ordinary benefits. Disclosure could carry workplace or immigration-related risks if confidentiality failed; anonymity cannot be guaranteed. Only privacy-reviewed group comparisons may be published.

These context questions are separate from the 50 NOSACQ-50 items. The optional birth year, gender and comment fields from the paper form are not collected.
