Hazard-based toolbox talk 10 of 52, with a practical HOP exercise.
Plan a 5–10 minute toolbox discussion, plus time to verify the actual controls. The one-page toolbox talk and editable Word version are public. Separate supervisor and attendance records are included with free Safety & Systems membership.
Why this matters
A repair can continue while the people change. Transfer the actual isolation status and each person’s protection before the incoming crew enters the work boundary.
Learning objective
Identify the applicable transfer procedure and evidence that protection continues through the handoff.
Key hazards or failure points
Unprotected incoming worker; unclear coordination; missing isolation information; stored energy returning; premature device removal; contractor mismatch.
Practical controls
- Use the equipment-specific procedure and applicable industry rules. Identify who coordinates groups, contractors and the shift transfer.
- Preserve required protection through the approved handoff sequence. Only designated trained personnel perform isolation actions and prescribed verification.
- Record work status, isolation boundaries, stored-energy controls and restrictions. Keep servicing separate from the authorization to restore equipment.
Verify It in the Field
- The coordinator reconciles actual equipment and protected workers with the procedure, isolation record and incoming work scope.
- Outgoing and incoming personnel review required verification results, continuing controls and unresolved restrictions from a safe position.
- Before exposure, the designated person confirms each incoming worker’s protection under the applicable arrangement; record completion, responsibility and time.
Stop and reassess
Hold for an unexplained device, changed scope, unknown worker status or a gap in protection. Keep clear and notify the coordinator. Resolve, verify and authorize the transfer before servicing resumes.
Crew discussion questions
- What protects each incoming worker?
- Which energy control needs continuing attention?
- Who can authorize the next step?
For each required control, record its acceptance source and criterion, safe verification method, responsible verifier, actual result and time. Mark VERIFIED, HOLD/NOT VERIFIED, or N/A with a reason. Complete all required inspections; never enter danger or improvise a test to fill out a form.

Turnover
Using the actual handoff record from a safe position, the incoming person explains the equipment state, remaining work and protection arrangement. The outgoing person resolves discrepancies. Name who confirms continuity and the point when responsibility transfers; do not manipulate devices for this exercise.
This exercise supports hazard identification or control verification. It does not replace safeguards, required training, specialist decisions or work authorization.
Leader preparation and practical detail
Establish applicability first
OSHA 1910.147 excludes construction employment. Its group and shift provisions, (f)(3)–(4), apply to covered general-industry work. Construction uses applicable activity-specific rules: 1926.417 addresses electrical circuits; 1926.702(j) addresses concrete/masonry equipment maintenance and repair. The employer must establish the controlling procedure for the actual activity.
Use the relevant transfer arrangement
For work covered by 1910.147, group procedures must provide the required personal protection, coordination and continuity; personnel changes require specific transfer procedures. Do not convert that into a universal construction lockbox sequence. Follow the actual employer procedure, preserving protection until the transfer is complete. No equipment-specific isolation procedure was supplied here.
Transfer evidence, not reassurance
Record which work remains, what is isolated, what can reaccumulate and which continuing actions remain necessary. The incoming person resolves conflicting records before accepting the work. Electrical testing belongs to appropriately qualified personnel under the applicable electrical procedure. Never remove another person’s device merely to make the handoff easier.
Short supervisor field check
- Applicable procedure identified
- Coordinator and groups confirmed
- Isolation evidence reconciled
- Transfer protection established
- Restart remains separately controlled
Use the separate sheet to document evidence, unresolved restrictions, correction owner, recheck and required release authority. Rebrief affected workers. This record does not replace specialist inspections, permits or engineering determinations.
Attendance and follow-through
Use the separate attendance record for participants, roles, questions, actions and late-arrival rebriefs. Attendance does not prove competence, functioning controls or authorization. Keep completed records private under employer procedures.
